📄 PUBLIC NOTICE OF VOLUNTARY TERMINATION
PUBLIC NOTICE DATE: 11/04/2025
This notice serves to inform patients, families, healthcare providers, and the general public that RELY HOME HEALTH (hereafter, “the Agency”) has voluntarily elected to terminate its participation in the Medicare Program in accordance with 42 CFR §489.52. The Agency will discontinue all Medicare-certified home health services effective 15 days from the date of this notice, or on the earliest effective date approved by the Centers for Medicare & Medicaid Services (CMS) and the California Department of Public Health (CDPH). The Agency has ceased operations and no longer admits or serves patients. All previously active patients have been safely discharged or transferred to appropriate alternative providers to ensure continuity of care. In compliance with federal regulations, medical records for all former patients will be securely maintained for the required retention period. The Agency appreciates the trust and support of the community and has taken all necessary steps to ensure a safe and orderly transition for all individuals previously under its care.
